Simply have a member of your quality department email your formatted spreadsheet (see below) to This e-mail address is being protected from spambots. You need JavaScript enabled to view it . The standard format is 6 columns of information. Direct your QA Manager to submit your information in the standard format in the following order:

Date Submitted, Part Number, Manufacturer, Date Code, Lot Code, and Findings/Comments.

The Findings/Comments column will typically be limited to the following explainations:

  • pulls
  • refurbished
  • solder on leads
  • bent/broken leads
  • tarnished/corroded leads
  • tinned leads
  • misaligned leads
  • substance on component
  • mishandled parts (supplier)
  • damaged packing
  • damaged tape/reel
  • missing characters
  • label marking different from part marking
  • ghosted/multiple markings
  • other
  • wrong packaging
  • wrong part number
  • pre-programmed
  • one time programmable
  • missing characters
  • wrong package (component)
  • failed functional spec
